1. Birth Certificate
2. Social Security Card
3. Immunization form (up-to-date) with parent's name and address
4. Proof of residency (CURRENT water, cable, power bill, etc.)
5. Parent ID (driver's license)
6. Custody Papers (if applicable)
7. Previous School Records and Transcripts
